Registration for our Summer Camp is a three-step process:
1) Complete and sign a Camp Registration Form

Each player/parent must complete and submit a camp registration form. Players under the age of 18 must have a parent or guardian fill out and sign the form. If you have not received one elsewhere, you may download a Camp Registration Form here.
IMPORTANT NOTE: This is a combination registration form, participation agreement, medical release and waiver of liability. It must be printed on two sides of the same sheet of paper. If you do not have duplex printing, please print page 1, then re-insert the page into your printer to print page 2 on the reverse side.

2) Complete a Player Profile Sheet

NEW PLAYERS: If you have never participated in a Northside Baseball Academy camp or team program, please download, print, and fill out a Player Profile Sheet. This can be sent with your registration and payment, or you can bring it with you on the first day of the camp.

3) Submit your Forms and Payment
|
|
